Adult Counselling Referral
If you would like to refer yourself or someone else to the Adult Counselling Service please complete the referral form below. If you are referring another person please ensure that you have their permission to do this before completing the form.
To refer to this service the person being referred must meet the following criteria:
- Be aged 18+
- Live in the Hart District OR be registered with a GP in the Hart District OR registered in our Family Counselling Service
- Must be committed to attending every week for up to 10 sessions
- Must not be receiving Counselling elsewhere
We will email the referrer to confirm receipt of referral within 5 working days. If you haven’t heard from us within this time please email 121@hartvolaction.org.uk to make sure we have received your referral.
