Referrals
How do I refer someone else?
If you are a health/social care/wellbeing professional or a family member/friend then you can refer someone else to Hart’s Community Garden. Please note you must have permission from the person you are referring before completing the referral.
- Fully complete the referral form
- You will receive an email confirming safe receipt of the referral within 3 working days.
- We will contact you if we require any further information.
- The individual referred (& the referrer if appropriate) will be invited along to an initial meeting with the Garden team this will take place either at the Garden or at our offices in Fleet.

Can I use my Direct Payment Budget?
Yes – if you have been assessed as being eligible by adult services and are in receipt of a ‘Direct Payment Budget’ then you can use this for a community garden membership. Please contact the garden team for more details.
What if I’m referred by someone else?
If you have been referred by someone else then you may have some additional support needs. Following an initial assessment with you, we can create a social and therapeutic session plan tailored to your needs.
