Oakmoor Referral Form

If you are a student at Oakmoor and would like to have some Counselling in school then please complete the form below. Your details will come directly to the 121 Schools Counselling Service and will not be seen by members of the school staff.

Once we are able to offer you some Counselling sessions we will ask Ms Riva to let you know the day and time of your sessions.

If you would like to make a referral on behalf of a student then please complete the referral form below ensuring you provide your details where it asks for details of the referrer.

If you are completing a referral on behalf of a student please ensure that they are aware of the referral being made.

The referral will come to 121 Schools Counselling and we will acknowledge receipt by email to the email address provided within 5 working days of receipt.

Once we have a slot for a student to see a Counsellor we will advise Lucy Riva of the day and time of the session and she will let the student know.
